Annapolis Asthma, Pulmonary & Sleep Specialists

If you are being seen for a pulmonary problem and have a chest x-ray or CT scan (at a facility other than Anne Arundel Diagnostics) within the past six months, please bring the film and the report with you. If you would like the doctor to address an existing condition for which you have been treated, please have your referring physician forward your recent medical records. They can be faxed to (443) 808-8551.

Please note that patients using a CPAP machine will need to bring a chip in with them.

Insurance

If your insurance company requires a referral please be sure to bring the necessary information at the time of your appointment.

Imaging Centers

We send patients to both Chesapeake Medical Imaging and Advanced Radiology. To review your imaging results, patients are able to bring us the Disc.
